First Time Evaluations

Most of our patients are referred by a medical professional. However, we do accept self-referred patients as long as you can provide us with background medical information. Please bring your insurance card to your appointment.

Physician Referrals

When we receive your referral from your physician, we will contact you to set up initial evaluation. We will ask to see any records of prior pain management treatments before setting up an appointment. After we set up an appointment, we will refer you to filling out new patient information forms prior to your appointment. Once we completely evaluate your situation, we will design a specific treatment plan for you. We offer complete diagnostic studies such as X-rays, MRIs, EMGs, and more to assist us in finding the primary pain cause. Additionally, we perform injections, such as nerve blocks, for diagnostic and therapeutic purposes. These are done in a short period of time under X-ray or ultrasound guidance, with minimal discomfort.

Maintaining Communication

Constant communication is always maintained with you and your referring and/or primary care physicians. We urge you to always keep your scheduled appointments in order to monitor your progress.
